Shockwave is required to annually report to the Centers for Medicare and Medicaid Services (CMS) certain information about payments and transfers of value provided directly or indirectly to covered recipients, as defined in implementing regulations. This includes any payments or transfers of value that Shockwave provides indirectly through third-party organizations to covered recipients, pursuant to this funding arrangement.
If the grant recipient is not a teaching hospital, the report to CMS includes the identity and business address of each relevant covered recipient and the value and purpose of any payments or transfers of value that are furnished. Grant recipients must track, maintain and provide to Shockwave information and data related to any payments or transfers of value that it provides to covered recipients under this funding arrangement in the form and manner that Shockwave requests in a timely manner. Shockwave may also report information about payments or transfers of value provided under this agreement as otherwise required by law and Shockwave reserves the right to post on a website accessible to the public such information, whether or not required by law.
If the recipient is a teaching hospital, Shockwave must also report any compensation, expenses, or other payments or transfers of value provided indirectly through grant recipients to U.S. physicians, physician assistants, nurse practitioners, clinical nurse specialists, certified registered nurse anesthetists, and certified nurse midwives pursuant to this funding arrangement. To enable Shockwave to comply with its legal obligations, grant recipients must track, maintain, and provide to Shockwave information and data related to payments and transfers of value that grant recipients provide to U.S. physicians, physician assistants, nurse practitioners, clinical nurse specialists, certified registered nurse anesthetists, and certified nurse midwives under this funding arrangement. This information about other compensation, payments or transfers of value may be posted on a website accessible to the public such information, whether or not required by law.
Templates for reporting will be available through the grants portal shortly.
Due to current limitations of the new portal, all organizations are referred to as nonprofits, regardless of their tax status. Educational grant applications may be requested by and awarded to both for-profit and not-for-profit/exempt organizations.

Shockwave provides funding to organizations or training institutions for scholarships to reduce the reasonable costs and expenses for Health Care Professionals-in-training (medical students, Residents, Fellows participating in Fellowship programs that are not-for-profit or have an academic affiliation).
Funding can be used to support individual Health Care Professionals (HCPs)-in-training (eg, medical and nursing students, Residents, and Fellows) salary or other indirect costs for one year. The names of individual HCPs-in-training benefiting from educational grants may not be known by Shockwave Medical in advance of the grant approval.
Travel scholarships grants in the form of travel and tuition may be considered to allow HCPs-in-training to attend a third-party educational program, provided that Shockwave does not select or control the selection of the specific HCPs-in-training who will benefit. Such travel grants will only be considered if submitted by the institution and not HCPs-in-training. Travel grants allow HCPs-in-training to attend a third-party educational conference where:
Funding should be used for individual Health Care Professionals-in-training, not to offset routine costs for Fellowship programs.
Each Application is reviewed on its own merits. It is recommended that the Application form be completed in its entirety, including documentation regarding the Fellowship Program, trajectory of Fellows post-program, and any data from Fellowships previously funded by Shockwave Medical.
